I worked for the City of Milpitas - Finance/Fiscal Services/Utility Section. I am doing research to implement some requirements of SB998 Policy on Discontinuance of Residential Water Service. I would like to request from City of Garden Grove copies of the following FORMS (in Excel or Word) on SB998 Alternative Payment Arrangements:
1) Form 998-A Certification of a Primary Care Provider
2) Form 998-B Certification of a Financial Hardship
3) Form 998-B2 Declaration of Household Income

I found these FORMS in your website in the PDF format. I am requesting these forms in either Excel or Word format.
