I am conducting a Phase I Environmental Assessment on the property address 12630 Westminster Avenue. I am requesting for any available records with the fire department for the property address in regard to the following:

* Underground/aboveground Storage Tank (UST/AST) removal or installation
* Lead-Based Paint (LBP) or Asbestos-Containing Material (ACM) abatement
* Hazardous releases or responses
* Environmental health code violations
* Water supply concerns

The City does not provide records on spills, leaks and clean-up, as that information is provided through the County of Orange Health Department.
