Please submit a Medical Release Form signed by the patient along with a copy of their I.D. to obtain a copy of the City of Garden Grove Fire Departments Prehospital Care Report. For your convenience, I have attached a copy of the City's release form.
The traffic collision report can be obtain by contacting the Garden Grove Police Department's Records Division at 714-741-5719.
Paramedic records can be obtained by contacting Care Ambulance at 714-828-7937.
