A medical release form must be submitted to obtain a copy of the Fire Department's Pre-Hospital Care Report. Attached for your convenience is a copy of the form. Please include a copy of the patient's drivers license and/or medical id.
To obtain medical and billing records please contact the hospital that the patient was transferred to. Copies of ambulance records can be obtained by contacting Care Ambulance located at 8932 Katella Avenue, #201, Anaheim, CA 92804.
