For insurance purposes I need a copy of an incident and medical report from May23rd, 2015, at or around 5:30 and 6:00 pm at the Village Green Park during the Strawberry festival. I fell and Paramedics were called to the scene. I need both the incident report explaining the reason for the call, and the report from the paramedics. I was told both were held with the city.
