As discussed a few minutes ago, since you have yet to receive the request from dispatch for the above, this is a formal written request for the 911 transcript.

To date the only records received from UCI Medical and Anaheim Fire & Rescue regarding dispatch reason (call disposition):
Prehospital Care Report: Completed
Dispatch Reason: Unknown Problem/Man Down

Thank you in advance for expediting this request as I'd like to forward this to my mom's Physician so he can treat her properly.
