1. Have you received test results for all the wells you were asked to test?
A: Yes.

2. Did any your wells reach 14 ppt for PFOA or 13 ppt for PFOS? If so, how many wells reached the threshold and what were the measurements for those wells?
A: There are 6 wells that reached the notification level for either PFOA, PFOS, or both. See attached results spreadsheet.

3. Have you notified the appropriate governing agency or agencies (cities and/or counties) of the exceedances? If so, what were those agencies?
A: Yes. They include the City of Stanton, the City of Santa Ana, the City of Westminster, and unincorporated Orange County.

4. Have you shut down any wells or other operations because of the test results? If so, please specify what has been shut down and whether that closure is permanent.
A: No wells have been taken out of operation due to PFAS results.

5. Have you notified customers, by either mail or email, of the exceedances? If so, please include a copy of the letter.
A: Customers have not been notified.

6. Have you made note of the exceedances on your website? If so, please provide a link to that notice.
