Hazardous Materials Incident Survey

The City of Garden Grove is seeking information from residents, businesses, and community members impacted by the recent chemical emergency and evacuation orders associated with the methyl methacrylate (MMA) tank incident.

The purpose of this survey is to help the City better understand the impacts experienced by the community and identify unmet needs related to displacement, housing, business interruption, and other recovery-related concerns. Information collected through this survey will assist the City in evaluating potential response and recovery resources, coordinating support services, and identifying areas where additional assistance may be needed.

If your household and/or business were impacted by the evacuation orders or related disruptions, we encourage you to complete this survey. Your feedback will help the City assess community needs and support ongoing response and recovery efforts.
